15. Abbreviated Checklists

Admission Checklist – this list applies to both new admissions and holdovers.

  • Confirm code status
  • Decide on level of care
  • Perform medication reconciliation
  • Confirm allergies
  • Add hold parameters to medications
  • Contact family
  • Contact PCP
  • Review final imaging reads
  • Review GI ppx, DVT ppx
  • Review IV access, need for urinary catheter
  • Update first call
  • Review rounding checklist

Rounding Checklist

  • Review labs, imaging, and microbiology data
  • Discuss overnight events with bedside nursing
  • Review medication list and usage of PRN medications
  • Review telemetry
  • Talk to consultants
  • Update patient and family regarding plan
  • If appropriate de-escalate lines (IV access and urinary catheter)
  • Decide whether daily AM labs are needed
  • Start discharge planning

Discharge Checklist

  • Review rounding checklist
  • Prepare medications for discharge
  • Arrange follow-up appointments
  • Arrange home health services
  • Ensure updated contact information for patient
  • Provide patient and family with education
  • Contact PCP to discuss plan and pending tests
  • Arrange for transportation
  • Complete discharge summary
